New
full time and new full time
seasonal employees are eligible
for coverage on the Company
group health plan on the first
day of the month following 30
days of employment. However,
coverage is NOT
automatic. This plan will be
effective on the first day of
the month following receipt (in
the Brooklyn office) of the
employee's enrollment form or
application. Each employee has a
120-day window to sign up.
Remember, the date on the
enrollment form or application
determines when your effective
date is. The first of the month
following ninety (90) days of
employment, the Company will pay
80% of the monthly group
insurance premium. The Company
share will be paid during the
layoff period after working two
(2) construction seasons. In
order for a new hire’s first
season to count as a full year,
they must have a hire date prior
to September 1st.
Therefore, a new hire that
starts before September 1st
will pay 100% of the premium for
only one (1), or the first
winter layoff period. Any
employee who quits and is
rehired will have the same
waiting period as a new
employee.
Returning full-time seasonal
employees, who previously waived
coverage under this Plan, may
enroll for coverage following
their rehire date. However,
application for coverage must be
made within the 30 day period. (For further
information on this item, please
see the Manatt's Inc. Group
Health Benefit book.) Certain
waiting periods and pre-existing
conditional clauses may apply.

Employees may choose to have
extra deductions made to cover
their share of the group health
insurance premium cost for the
layoff season or slow periods.
New employees who enroll in
Manatt’s Group Health Plan as
soon as they are eligible, may
sign up for the Cafeteria Plan
for PREMIUM ONLY. Returning
full time and full time seasonal
employees must sign up by
January 1st each
year. This allows them to pay
their share of the health
premium with pre-tax dollars.
In
order for an employee to remain
eligible for the Company’s share
of the premium, they must be a current full time
or full time seasonal employee.
The maximum months covered
during a layoff will be six (6)
months.
Employees who are included in
their spouse’s group health
insurance plan may be eligible
for a contribution by Manatt’s
into the Cafeteria Plan. See
CAFETERIA-FLEXIBLE BENEFITS
PROGRAM.

Following are the conditions of
our FIRST ADMINISTRATORS group
plan:

Effective April 1, 1998, and
thereafter, new employees
and late entrants will have
a waiting period for
pre-existing, existing
conditions and obstetrical
care as defined below.

With a Certificate of
Creditable coverage
(month for month),
pre-existing conditions
could be eliminated.
Waiting
period for pre-existing and
existing conditions
Until this and prior
Agreement(s) issued by The Plans
have been in force for nine (9)
consecutive months, The Plans
will not pay for:
-
Any illness, injury or other
condition existing on the
effective date of this
Agreement or which
progressed or developed from
or was a complication of or
secondary to any illness,
injury or other condition
existing on such effective
date; or
-
Any illness, injury or other
condition for which medical
or surgical treatment or
advice was rendered within
six (6) months prior to such
effective date.
